On Friday, May 23rd, the Lawrenceburg Sheriff’s Department responded to a call involving forgery. Deputies spoke over the phone with a TDOC member about an individual who turned in a medical document that had been altered. The member spoke with a member from the District Attorney’s office and forwarded an email showing the altered medical excuse. Deputies then went to Fast Pace Medical in Lawrenceburg where they spoke with a Nurse Practitioner who stated he did not write the extra medical issues that were on the note that the individual turned into the Probation and Parole office in Lawrenceburg. The original copy of the medical excuse received from Fast Pace showed no other medical problems for the individual. The forged note stated the individual had numerous medical problems. It was then decided the individual did forge the document after receiving it from Fast Pace. A warrant was then issued by the Lawrence County Sheriff’s Department.
