Meadowbrook Healthcare and Rehabilitation Center, an 83-bed skilled nursing facility located in Pulaski, Tennessee.

Wellpark Health and Rehabilitation, and Legacy Park Health and Rehabilitation, both located in Knoxville.

VanAyer Senior Living and Rehabilitation, located in Martin.

Union City Health and Rehabilitation, located in Union City.

Decatur County Healthcare, located in Parsons.

Savannah Nursing and Rehabilitation, located in Savannah.

Westwood Nursing and Rehabilitation, located in Decaturville.

The Health Center at Research Park, located in Huntsville.

Ensign-TM and its subsidiaries completed the acquisitions on December 31, 2024. All nine operations are operated by Ensign affiliated operating companies.
